Creating a form in JotForm

AppointmentGuru is committed to continuously finding ways to make your life easier. We have recently introduced a new integration with JotForm. This integration allows your clients to fill in all the forms you need from them easily, and from their own devices. Once the clients have completed the forms, the forms are immediately added and stored to their client profile on your AppointmentGuru profile. How great is that. No more paperwork or filing!

First you need to signup and create your own JotForm account. You can do that here: https://www.jotform.com/login/

Creating a form in JotForm

Here is where you will upload all the blank forms that you want your clients to fill in. We advise converting them to PDF’s before uploading them. It makes your life a little easier when you format them. * In your JotForm account go to the MY FORMS page. * Here you are able to import the forms that you wish to link to your AppointmentGuru account. (We suggest importing PDF documents because it makes formatting them a little easier for you.) * Once you have imported the forms that you want to use, you can start converting them by clicking EDIT FORM. * Once you have done this the form builder will open. Here you can build the form to suit how you wish to capture your client information. * When you have finished you click PUBLISH. Here you open the JotForm link and check you are happy with the format of the form by downloading the PDF after you have filled it in. * The form has been added to your profile and can be found in MY FORMS. (If you want to edit the format, you can do so by clicking EDIT from the MY FORMS page) * If this isn’t clear, here is a video guiding you each step of the way if you need it: https://www.youtube.com/watch?v=wqQSLnrjngQ&feature=youtu.be
