Creating a form in JotForm
Two options:
- Create a new form by dragging and dropping form elements. Read more on How to create your first form.
- If you have existing forms in a PDF format, import them. Read more on How can I import my own PDF. Once imported, edit them so that they're formatted to your requirements.
Config to remember: * The form must contain a hidden field named relationship_id. This field is crucial as it passes the client's ID to the form submission, enabling AppointmentGuru to link it to the client's profile. * Add a short text field to the top of the form with the name relationship_id * In the Properties of the field, click on 'Advanced' and make sure 'Hide field' is on. * Jotform provides a full name field which looks like two separate fields, it's synced to AppointmentGuru as one field and breaks the form submission. Rather use individual short text fields for first name and last name. This will be easier to map to your profile in AppointmentGuru.
Once your forms are created, make sure they're published.
