Link your forms and map fields
For JotForm to send the form submissions to AppointmentGuru, you will need to link the form and optionally map certain fields from your form to corresponding fields on your client's profile
Steps to follow
1. Link your form
In AppointmentGuru from the JotForm integration page
- Click "link" next to the form you would like to connect
- In the resulting window, choose the spaces with which you would like to use this form and optionally select to add it to your notifications ("Link this form").
- Click the "link" button
This will add a webhook integration to your form so that when a client submits it, the submission will be sent to AppointmentGuru as well as saved in JotForm.
Note: You can edit form settings (space and adding to notifications) by clicking "settings" from the form menu
2. Map fields
Once you have linked the form, you have the option to map fields in your form with fields on your client's profile.
You can use this to automatically update information on the client's profile (e.g.: contact details, medical aid information) based on the information they submit in the form.
To map fields:
- From the dropdown on the form titled "Map your form fields to your client's AppointmentGuru profile"
- From the left column, choose the field on the client's AppointmentGuru profile
- From the right column, choose the field from your form
- Click "save"
- The list of fields that are mapped is shown in the section under the heading "these fields are mapped"
- With mapped fields, you can also quickly make them required
Important: if you have your form open in JotForm separately, make sure to refresh the page after making the above changes.
How this works:
When you map a field, AppointmentGuru will update the unique name of the field in JotForm. When the submission is received from JotFrom, AppointmentGuru looks for appropriately named fields and will map those fields to the corresponding fields in the AppointmentGuru database.
