How the JotForm integration works

AppointmentGuru's JotForm integration allows you to design forms of any type in JotForm. In JotForm, you can create consent forms, patient information forms or even surveys.

Link these forms to your AppointmentGuru account and allow AppointmentGuru to include a link to your forms in your notifications.

Once successfully integrated, the flow works as follows:

  1. An appointment is created in AppointmentGuru (either through a client self-booking online, or one created by you).
  2. A link to your forms is included in the automatic confirmation notification that is sent to the client or you can share a link to the form with your client directly.
  3. Clients complete the forms online and submit.
  4. The submission is sent to AppointmentGuru and linked to the client's profile. Correctly mapped fields from the form (such as contact details and medical aid information) will be updated in the client's profile.

Setting up the integration (links to specific help pages below):

  1. Create your Jotform account and forms.
  2. Connect AppointmentGuru and Jotform.
  3. Link the forms and map fields.
  4. Update appointment types in your AppointmentGuru account.
  5. Include form links in your automated notifications.
