How the JotForm integration works

A quick overview showing you how the JotForm integration works from your client filling out the forms online though AppointmentGuru connecting the form submission to their profile

Our JotForm integration allows you to design forms of any type in JotForm. In JotForm, you can create consent forms, patient information forms or even surveys.

Link these forms to your AppointmentGuru account and allow AppointmentGuru to include a link to your forms in your notifications.

Once successfully integrated, the flow works as follows:

  1. An appointment is created in AppointmentGuru (either through a client self-booking online, or one created by you).
  2. A link to your forms is included in the automatic confirmation notification that is sent to the client
  3. They click on the link and fill in the forms online
  4. When they submit a form:
    1. the submission is sent to AppointmentGuru
    2. AppointmentGuru will link the form submission to the client's profile
    3. Correctly mapped fields from the form (such as contact details and medical aid information) will be updated in the client's profile
